Montek LC vs Montair LC: Which Allergy Combo Is Better?

BBoth contain Montelukast 10mg + Levocetirizine 5mg. Montek LC (Sun, ₹125/strip of 10). Montair LC (Cipla, ₹140/strip). Also Odilast LC, Telekast L, Lukotas LX - all same combination. Montelukast is a leukotriene receptor antagonist - reduces inflammation in airways and nasal passages. Levocetirizine is an antihistamine - blocks histamine for runny nose, sneezing. Combination targets both asthma/allergic rhinitis mechanisms. Same effectiveness across brands - pick based on availability and price.

Allergic rhinitis (seasonal or perennial): Nasal congestion + sneezing + runny nose + itchy eyes. Mild to moderate asthma (adjunct): Not a primary inhaler replacement. Exercise-induced asthma (take 2 hours before exercise). Chronic urticaria (hives). Post-viral airway hyperreactivity. Typically prescribed for: Allergy season (Diwali-winter, monsoon season in India), when inhaler + SOS salbutamol isn't enough. Dose: 1 tablet at night (montelukast preferred evening timing). 4-12 weeks course or seasonal use.

Montelukast syrup or chewable: 4mg for 2-5 years, 5mg for 6-14 years, 10mg for 15+ years. Levocetirizine 2.5mg for 6 months-5 years, 5mg for 6+. For kids, doctor may prescribe these separately: Montek 4 chewable + Incid-L 2.5 syrup. Combination (Montek LC) generally for adults/teens. Given before bed for best effect on morning symptoms. Take regularly during allergy season - on-demand doesn't work as well for chronic allergy.

FDA-level warning on montelukast: Mental health side effects - new agitation, anxiety, depression, nightmares, suicidal thoughts (rare but serious). Especially kids. If child develops mood changes / nightmares on Montek LC: stop, tell doctor. Other side effects: Headache (10%), stomach upset (5%), dry mouth (8%), fatigue, eosinophilia (rare - lung inflammation, can mimic asthma worsening), Churg-Strauss syndrome (rare vasculitis). Drowsiness from levocetirizine component. Vivid dreams. Rare: liver enzyme rise.

Phenobarbital, rifampin: Reduce montelukast levels. Warfarin: Monitor INR. CNS depressants (sleeping pills, alcohol, benzos): Worse drowsiness from levocetirizine. Alcohol: Avoid - drowsiness + may worsen mood effects. Moderate interaction with certain anti-epileptics. Few significant interactions otherwise. Food: Can be taken with or without food. Consistency matters for montelukast - take at same time daily.

Severe persistent asthma: Needs ICS (inhaled corticosteroid) like Asthalin or Budecort inhaler - Montek LC alone insufficient. Daily nasal symptoms despite Montek LC × 4 weeks: Add nasal steroid spray (Flomist, Nasonex) + evaluate for structural cause (deviated septum, polyps). Anaphylaxis: Needs adrenaline, not Montek. Food allergy: Identify + avoid trigger primary; Montek is adjunct. Chronic cough > 3 weeks: See doctor; may be asthma variant, GERD, or post-nasal drip - targeted treatment better than blanket Montek LC.
